"Your iron is fine" almost always means your hemoglobin is fine — not your iron stores. The two are different tests, and they empty in a specific order: your stores run down first, and your hemoglobin holds steady until they're nearly gone. That gap has a name — iron deficiency without anemia — and it's roughly twice as common as the anemia most doctors are screening for. The marker that reveals it is ferritin, and on a standard "your bloods are normal" visit, it's frequently never run. So you can be genuinely low, genuinely exhausted, and still be told everything looks fine.
Ferritin vs hemoglobin: the part almost everyone gets told wrong
Think of your iron as a bank account. Hemoglobin is the cash in your wallet — what's actively circulating in your red blood cells right now. Ferritin is your savings — the iron your body has in reserve. When iron runs short, your body drains the savings account first to keep the wallet full, because circulating oxygen is non-negotiable. So ferritin drops long before hemoglobin does.
This is why the sequence matters. Iron deficiency isn't a switch that flips at anemia; it's a slope. First your stores fall (low ferritin, normal hemoglobin) — you feel it, but the CBC looks clean. Only later, when the reserve is genuinely gone, does hemoglobin finally drop and the word "anemia" appear on a report. By then you've often spent a long time being told nothing is wrong. You are deficient before you are anemic.
And the symptoms show up in that early, "normal-CBC" window: fatigue out of proportion to your life, breathlessness on stairs you used to take easily, brain fog, hair shedding, cold hands, restless legs, a heart that races for no reason. None of it is imagined. It's your reserve running low while the one test that would show it goes unordered.
The driver underneath: nutrient status, not willpower
Iron isn't only about energy — it's a raw material your reproductive system draws on. Every cell that divides quickly needs it, and few tissues divide faster than a developing follicle and, later, an early pregnancy and its placenta. Iron supports the oxygen delivery those tissues run on. When stores are chronically low, the body does what it always does under scarcity: it protects the essentials and deprioritises the rest — and reproduction is something the body treats as optional when resources are tight.
The observational signal points the same way. In the Nurses' Health Study — a large prospective cohort — women with a higher intake of non-heme iron (the plant and supplement form) had a meaningfully lower risk of ovulatory infertility than women with the lowest intake, and regular iron-supplement users had a lower risk than non-users. Separately, women with recurrent pregnancy loss have been found to carry lower ferritin levels on average than women without loss. The thread running through it: iron status appears to sit upstream of ovulation and early pregnancy, not off to the side of them.
I won't oversell this. The Nurses' Health Study finding is observational — it shows an association, not proof that iron pills cause pregnancies — and it has not been consistently replicated; later studies found no clear link between iron intake and time to pregnancy. There is also no single agreed "fertility ferritin" number: labs often flag deficiency only below ~15 µg/L, many clinicians treat symptoms below ~30 µg/L, and some frameworks call anything under ~50 µg/L "insufficient" — the optimal target for conception specifically is not a settled guideline. So treat ferritin as a genuinely useful, cheap, modifiable thing to measure and raise if it's low — not as a magic lever. The point isn't a number that guarantees anything; it's a common, fixable deficit that usually never gets checked.
Why this matters when you're trying to conceive
Three practical reasons. First, it's common and it's cheap to check. A ferritin test costs very little, and low stores are one of the most prevalent, most correctable nutrient gaps in women of reproductive age — especially anyone with heavy periods, a plant-forward diet, or a previous pregnancy that drew down their reserves. Second, pregnancy raises the demand sharply. Iron requirements climb across pregnancy; going in with an empty tank means starting a marathon already depleted, which is linked to fatigue and worse outcomes for mother and baby. Building the reserve before conception is easier than scrambling during it. Third, it explains a symptom you've been gaslit about — the bone-deep tiredness that "normal bloods" never accounted for.
None of this means low ferritin is why you're not pregnant. It means it's a driver worth mapping and correcting — a real, modifiable input — rather than one more thing quietly draining you in the background while everything "looks fine."
So what should I actually do?
Not panic, and not start megadosing iron off a Reddit thread (see the warning below). The move is one specific test and a calmer conversation. You might ask your provider some version of:
"Can we check my ferritin — not just hemoglobin — and a full iron panel? I've got the fatigue / hair shedding / breathlessness pattern even though my CBC is normal, and I want to know whether my iron stores are low before we keep trying, since pregnancy raises the demand."
If your ferritin comes back low, here's what's reasonable — and where to be careful:
- Test before you supplement, and re-test after. Iron is one of the few nutrients you can genuinely overshoot; dosing blind is not harmless. Confirm you're low, correct it, then re-check in ~8–12 weeks to see the number actually move.
- Look for the why, not just the low number. Heavy periods are the most common cause in this group and are themselves worth a conversation. Low ferritin is a clue about the whole picture, not just a box to top up.
- Food and absorption matter. Pair iron-rich foods with vitamin C; be aware that tea, coffee, and calcium with meals blunt absorption. Your provider can advise whether food alone is enough or whether a supplement is warranted, and at what dose.
- Severe or stubborn cases (very low ferritin, poor absorption, or no response to oral iron) are a medical conversation — sometimes IV iron — not a supplement-aisle decision.
Unlike most nutrients, the body has no easy way to excrete excess iron, and overload carries real risks. Never megadose iron on the assumption that "low is bad, so more must be good," and don't take a fertility iron supplement long-term without confirming you're actually deficient and re-checking your level. If you have a family history of haemochromatosis or any iron-overload condition, iron supplements can be actively harmful — test first. This is exactly why measuring the driver matters before you change anything.
The bigger principle: normal isn't optimal
Low ferritin is one of the clearest examples of the thing this whole site keeps returning to: a "normal" result is the band where most of a population falls — it was never calibrated to the question "what does this woman's body need to conceive and carry well?" A normal hemoglobin, a normal-looking CBC, a normal visit can all be true while your iron savings sit near empty. The exhaustion no one can explain is usually not unexplained — it's un-measured.
